I am a registered nurse of over 10 years turned nurse esthetician, and I am very excited to do your lashes! ***Please note that to ensure you receive the best quality service, I do not apply lashes over someone else's work. If you come to your appointment with lashes on, you will be charged a $15 removal fee.

By booking my services, you consent to the following:

1. I authorize my lash technician to take pictures and/or record videos of my eyelashes before, during, or after the service.


2. I agree to keep my lash technician informed if any discomfort, irritation, or dissatisfaction occurs during or immediately after the set.


3. I agree to follow the recommendations of my lash technician in terms of the use of compatible products, maintenance and care of my natural eyelashes, and lash extensions. I release my lash technician from all liability if I do not follow these same recommendations.


4. I understand that there are certain risks associated with eyelash extensions, such as allergy, redness, or irritation, and that my lash technician cannot be held responsible if a reaction occurs during or after the set.


5. I understand that several factors can affect the hold or appearance of eyelash extensions after application, such as dirt/makeup buildup, temperature changes, hormonal variations, or medication.
