To ensure every patient receives the highest quality of care and our undivided attention, we ask that you review our clinic policies below:

• Securing Your Appointment: All appointments require a brief phone call to set up your profile, send your medical intake forms, and redeem your Groupon voucher to secure your spot.

• Paperwork & ID: Because we are a medical practice, we must have your completed intake forms and a copy of your photo ID on file before you arrive. If these are not completed prior to your visit, we will unfortunately be unable to treat you, resulting in the forfeiture of your appointment without a refund.

• Cancellations & Rescheduling: We kindly request at least 48 hours' notice for any changes to your appointment. Cancellations made with less than 48 hours' notice will result in the forfeiture of your visit without a refund, or you may choose to pay a rescheduling fee to book a future appointment.

• Running Late: If you are more than 10 minutes late, your provider will evaluate whether there is still enough time to safely perform a shortened version of your service. This is at the provider's sole discretion. If we cannot accommodate the shortened time, the appointment will be forfeited without a refund.

• Mutual Respect: We are committed to providing a kind, safe, and welcoming environment, and we ask for the same in return. Aggressive, rude, or disrespectful behavior toward our staff will not be tolerated and will result in an immediate refusal of service and forfeiture of your appointment without a refund.

• Communication: When we are with a patient, they have our complete focus—and you will receive that exact same dedication during your visit! Because of this, we cannot always answer the phone. Please leave a voicemail. We do not answer blocked/unknown numbers, and we cannot return calls that do not leave a message.

• Treatment Results: Great results take a little time! Please allow the recommended timeframe for your specific treatment (such as neurotoxins) to fully take effect. If you have any questions during your healing process, please call the clinic or reach out to your nurse directly.
