Paperwork & ID: Because we are a medical practice, we must have your completed intake forms and a copy of your photo ID on file before you arrive. If these are not completed prior to your visit, we will unfortunately be unable to treat you, resulting in the forfeiture of your appointment without a refund. Cancellations & Rescheduling: We kindly request at least 48 hours' notice for any changes to your appointment. Cancellations made with less than 48 hours' notice will result in the forfeiture of your visit without a refund, or you may choose to pay a rescheduling fee to book a future appointment.
