A valid Ontario Health Card is required to book an appointment. The medical history form is sent to patients online prior to their visit and must be completed in advance; our Medical Director will then review the file to confirm there are no contraindications before treatment. If the form is not completed, the appointment will be canceled. Vouchers must be redeemed by the original purchaser, and if purchased as a gift, proof of purchase is required.
