This extra charge will not be made by groupon but directly to the hotel. The fine print reads '4-day cancellation notice required or fee of applicable two-nights' stay up to the amount paid for Groupon applies.' Does this mean I have four days from the purchase date to cancel? And, if so, will I receive a full refund? What if the hotel does not respond to my reservation request within the four days, letting me know if the dates I would like are available?

There is no shuttle service from the airport to the hotel. You can take a taxi at the airport, it's approx USD 25 from the International Puerto Plata Airport.
