Amount paid is the amount returned to the patient in lieu of redeeming the voucher if, upon presentation and consultation, the patient is not eligible to receive professional services advertised in the offering. The Smile Protection Plan (SPP) is not insurance. This is a licensed discount dental plan. SPP provides discounts only at Great Expressions Dental Centers for dental services. SPP does not make payments directly to the providers of dental services. You (member) will be required to pay for all health care or dental services but will receive a discount from Great Expressions Dental Centers, who have contracted with The CDl Group, Inc. Members will have 30 days after the effective date of enrollment to cancel their plan if they have not utilized and received discounts under the plan. Voucher must be present at time of office dental visit. One take-home whitening kit per Smile Protection Plan voucher purchase, if applicable. This voucher is only redeemable at participating Texas-Area Great Expressions Dental Centers locations. Licensed and operated under Dr. April Kaneira. Value of savings for exams and X-rays with the Smile Protection Plan for 18 months are based on one examination every 6 months per individual. Value of savings is used for illustration purposes only. Actual savings may vary depending upon frequency of use. Six-month extension begins immediately after the end of the 12-month plan. Appointment required. Consultation required; non-candidates and other refund requests will be honored before service provided. Not valid with insurance. Limit 1 per person. Valid only for option purchased.
