In addition, patients will have to undergo a medical evaluation at our office to determine eligibility. The medical evaluation is know as Good Faith Exam and will be completed prior to any service to comply with CA rules and regulations. Patients will be responsible for a one time fee of $35 to covers the GFE (Good Faith Exam).
