Yesterday, Jan. 29th, I filled out the Groupon form to buy a groupon for my granddaughter to McCollum Wellness center. I paid for it with my credit card. So far, I have received nothing back, no confirmation number, etc. My name is David Meacham and my granddaughter's is Rebecca Roth. My email address is email@example.com. Hi David, all you need to do is print out your groupon reciept or come in with the groupon identification number then call my office for an appointment, mention that you bought a groupon. Thanks Dr Duncan McCollum DC 831 459 9990
