Appointment required. Must sign waiver. Consultation required before service. If you are ineligible, a refund will be provided. Customers may be asked to leave a credit card in order to book an appointment. In the case of a "no show" the patient will be charged a $150 fee. Patients will have 48 hours to cancel or reschedule their appointments. Appointments cancelled hours prior or the same day may be subject to the $150 fee. For the options marked "Restrictions apply"; Limit 1 per person per service/product, May be repurchased every 365 days.
