Outlier Health and Wellness is a in-person and telehealth medical practice that provides competent medical care in the patient's homes as well as virtually. We specialize in providing primary care for the elderly and chronic care homebound patients.   We provide transitional care visits for those patients recently discharged from a hospital or facility. This service allows patients to have a medical provider care for them in their homes until they are stable enough to continue outpatient visits with their primary care provider. Telehealth visits are available for patients that are stable and need follow-up on a chronic condition. We also provide non-emergent and uncomplicated visits.   Our certified providers offer certifications for MMJ cards to qualified eligible patients living in New York State.
