What to Expect

1) Customers will schedule an appointment with the team by calling (833)-245-3239 or visit https://www.md.leafyquick.com/

2) Schedule a telemedicine Appointment. Meet at the appointment center on the date and time of the appointment.

3) Customer will get an evaluation by the physician and receive their medical card within a few days if they qualify. If the customer does not qualify, they will be reimbursed.

4) Bring the voucher to the medical certification appointment. 

5) The merchant will help the customer fill out the Illinois Department of Public Health Application on the spot. 

State fees are as follows: $100 for 1 year card, $200 for 2 year card and $250 for 3 year card).

Q: What other requirements are there to get a card?

A: You must be able to demonstrate that you are an Illinois resident, with a driver’s license and official mail in your name sent to your current Illinois address (both need to match).

Q: How long does it take to get a card?

A: You will have your card within several days after your appointment.

Q: What if I am not sure if I qualify?

A: Give us a call to confirm, but make sure to bring along your medical records and any medication you are taking and let’s figure it out together.
