Doctor conducts consultations to determine the patient’s eligibility for medical marijuana certifications
THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAVE A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
Requirements: Valid Florida’s Drivers License or ID with current address. Snowbirds: 2 separate proof of FL residency Medical Records stating qualifying condition. Records cannot be older than 2 years from the time of appointment. Records DO NOT include medications, medication lists, any type of imaging (MRI’s, Biopsies, etc. ), MRI readings, or lab results.
Additional Fees: The Florida Department of Health has a fee of $77.75 for the state medical marijuana card. Keep in mind this is a separate fee from the office fee. This is also a yearly fee.
Follow-ups: Re-certifications are required every 210 days (approximately 7 months). The fee is $149 every 7 months.
Availability for appointments: Wednesday’s from 1 pm-5 pm. Every other Friday from 9 am-5 pm.
