I didn't use the coupon, because they said I bought phase III of the cleaning service. Therefore, I paid the phase I in cash. The weird thing is that Carolina Dental Arts redeemed the coupon, which should be used for "phase III" and asked me to come back later. I called for a refund, but no response yet.
