Hi Ana, You were charged $26.18 not $26.90 which is the tax of the original price of the service you purchased to try, Groupon service are added on a discounted value for the clients to try and see if they will like to continue with it, not as the actual price of the service. In the State of New York tax must be charged based off the original price with Groupon which we showed you proof of as well and this is mentioned before the clients session as well. Your service was in total 1hr and 15 minutes together with skin analysis. The form is just the facial consent form which all clients are require to fill out in order for the aesthetician to give you a more personalize treatment tailored to your skin. When booking an appointment we hold a temporary deposit in case of no-show or late cancellation which is refunded once the client arrives for their session. We are sorry you felt the service wasn't up to your standards and we appreciate you choosing us for your skincare needs.
